Understanding Nephrostomy Procedures and How to Care

Nephrostomy is a procedure performed to drain urine from the kidneys through a catheter. This action is carried out if there is a blockage in the ureter, which should function to drain urine from the kidneys to the bladder. Nephrostomy is generally done when there is urine obstruction due to urinary tract infections, kidney stones, tumors, leakage or damage to the organs that pass urine (generally due to congenital anatomical abnormalities, physical injury, inflammation, and cancer), or related to pregnancy. In addition, nephrostomy can also be used as a pathway to assist other medical procedures, both for diagnostic and therapeutic purposes.

Nephrostomy Procedure

Before undergoing a nephrostomy procedure, the doctor will conduct a thorough examination, and assess your condition. The examination also includes the history of the treatment you are currently undergoing, because there are several types of treatment that need to be stopped before undergoing nephrostomy. Your doctor may also advise you not to eat for at least 4-6 hours before the procedure. Don't worry, the doctor will give an anesthetic injection or anesthetic, so that you don't feel pain during the procedure. This procedure will use the help of an ultrasound or CT scan, so that the catheter can be placed in the right position. Once installed, the catheter will be connected to the urine bag. Nephrostomy procedures generally require approximately 60 minutes. Channels made in this procedure, will be maintained for a certain period of time, according to your physical needs and conditions. Can be only a few days, or even months. For this reason, proper care is needed to maintain the quality and function of nephrostomy.

How to Care for Nephrostomy Hose

After the nephrostomy procedure has been performed, the doctor will explain how to treat the nephrostomy tube, there are several things that must be considered in treating a catheter. When you want to check the condition of the hose and nephrostomy bag, you are advised to pay attention to several things, namely:
  • Make sure the bandage is in a dry, clean condition and in its proper position.
  • Note the condition of the skin around the nephrostomy tube area, whether there is a skin rash or redness.
  • Pay attention to the urine in the bag, check the color and empty if it is full.
  • Make sure the hose is not folded or twisted and impedes urine flow.
The following steps are general instructions for caring for safe urine hose and drainage bags:
  • Replace the bandage, skin barrier and attachment device according to instructions. At least every 7 days.
  • Nephrostomy tubes need to be replaced according to the time period they have. For long-term nephrostomy tubes, the tube may need to be replaced every 2-3 months.
  • Wipe the connecting end of the drainage bag with alcohol or povidone iodine before you reconnect the drainage bag to the hose
  • If the drainage bag is full, empty it, and replace it with a clean bag.
  • Make sure the catheter hose stays connected to the drainage bag, and that it sticks well.
  • Use a larger drainage bag at night and during a nap, so it can hold more urine.
Immediately consult a doctor, if you feel back pain or body side that does not go away or getting worse, blood in the urine, fever, vomiting, urine smell, or pain in the skin around the catheter tube, after the nephrostomy procedure.
